Participation

You are not required by the Shingo Institute to participate in this survey. You may withdraw from this survey at any time during the survey by simply closing the survey window. If you intend to complete this survey, you will be required to answer all questions on a page before moving to the next page. Incomplete surveys will not be tabulated into the results and the data collected will be removed from the system at the end of the survey period.
